Pathways Program Overview
The Caddo Nation received a Pathways to Community Living Grant from The Oklahoma Healthcare Authority for a Pathways Program beginning October 15th, 2024.
The Pathways Program provides a comprehensive holistic quality service that is unique to the Caddo Nation tribal members, other tribal affiliation members, or community members that are Medicaid recipients. Our goal to provide services to enable Elders to maintain in their homes or return home from skilled nursing facilities, and to be able to help others who are in need and qualify for Medicaid to receive these services.
Members do not need to reside within the Caddo Jurisdiction, and it is open statewide.
Services
- Overall Health Assessment
- Assistance with distribution of medication
- Wound care
- Routine hygiene,
- Light house duty
- Meals
- Transportation to and from doctor visits or to a Caddo Nation facility for use of Telehealth
- Errand running
- Telehealth is available at two locations: Caddo Nation AOA in Binger, OK and Social Services Community Health Building in Anadarko, OK.
