The Caddo Nation Community Health Program Mission is to assist eligible Native American Individuals and families within our service area to gain access to comprehensive health care services. Our program is funded by IHS on a yearly basis to assist Caddo tribal members with health needs. We are often a vital link to health care services for our communities in rural areas.
Our work includes scheduling appointments for our clients, pick up and delivery of medications, home visits, case screening and management, monitoring of patients, and health education. All of our CHR staff members are trained in CPR, First Aid and as EMS First Responders.
Transportation to outside referrals is provided if the appointment has been made by an IHS doctor.

The staff transports clients who are without transportation to doctor’s appointments at the Anadarko IHS clinic or the Lawton IHS hospital, El Reno or Clinton. We can also provide transportation to outside referrals made by a doctor from IHS. The CHR Office needs to know 2 to 3 days in advance about appointments, refills of prescriptions, and transportation needs.

The CHR and Special Diabetes Program Hosts a Podiatry Clinic twice each month at our offices for both Caddo tribal members enrolled in our on-going podiatry program, and members of other Federally recognized tribes. Shoes are available once annually after a client has been participating in the podiatry clinic.
2-3 DAY ADVANCE NOTICE REQUIRED
The CHR Office needs to know 2 to 3 days in advance about appointments, refills of prescriptions, and transportation needs. There is a call-in procedure for requesting medication at the Lawton Indian Hospital; call toll-free 1-888-275-4886 for assistance, and allow 2 days before picking up medicines.
MEDICATION REFILLS AT IHS CLINICS & HOSPITALS
The I.H.S. clinics and Lawton Hospital are computerized and if you have refills on medications, they can now be filled at the Anadarko Pharmacy. You may call them in and let us know if you need a CHR to pick them up for you, or let us know and we can request them for you.
ANADARKO/CARNEGIE/LAWTON SAME DAY APPTS
Anadarko & Carnegie clinics and Lawton Indian Hospital are doing same day appointments. You must call in each morning starting at 7:45 a.m. to get an appointment for that day. Any further questions please call the Community Health Office.
The Caddo Nation Diabetes Program is administered through the CHR Office.
